• Provide non-profit 501 (c) (3) organization documentation to show the agency is a non-profit organization;
• Provide job-readiness preparation and training to their clients and provide documentation that client has completed the training;
• Be able to send one or more agency personnel to an Annual Agency Meeting held every January;
• Submit the CGH Agency approval form that has addressed all the areas of information and signatures requested.

If you wish to become an approved agency to refer your clients for service, please begin the process by emailing us at This e-mail address is being protected from spam bots, you need JavaScript enabled to view it   Your request should include preliminary information regarding your organization such as organization name, contact person(s) and title(s), address, phone, fax, primary delivery services, how many clients you expect to send us annually, and how did you hear about us.  

Once agency is approved, they can then:

• Submit a referral form for each client requesting an appointment.  Remember, each client must  have completed a job-training program and is in active job search with an upcoming appointment for a job interview or will be attending a job fair within the month.
• Explain to referring clients that they need to respect and adhere to CGH rules for appointments with our organization such as be on time, call ahead if they plan to be late, and notify us at least 24 hours if they must cancel and reschedule their appointment.  We also expect the client to maintain the highest personal hygiene, as if they were going on their interview.  If client fails to respect the time of our staff and volunteers, as well exhibit good hygiene, we will reserve the right to decline services.
